Intent to Donate Investment Securities to FPC

If you plan to contribute shares of securities (stocks, bonds, ETFs, Mutual Funds, etc.) from your investment accounts, please fill out this form and click submit to inform the FPC Church Administrator of your intention.  You will still need to initiate with your broker transfer of the securities, but this will allow the FPC Church Administrator to track their deposit to the FPC account.
 
Upon receipt of this form, the FPC Church Administrator will provide you with detailed information for your broker about where to transfer the securities.
